Terms and Conditions
By signing this document you conform and agree to the following
  1. You acknowledge and agree that there are risks and dangers inherent to the participation in the school and its related activities such as exposure or infection by contagious diseases including but not limited to COVID-19, sports, gym, tumbling, and transportation such as carpools and bussing, including walking to the school from the designated parking areas for carpools. These risks and dangers include, but are not limited to broken bones, sprains, strains, cuts, bruises, sunburn, sickness, disability, and/or death. By enrolling the above child or children and participating in the school and its related activities, you agree to accept all risks and dangers inherent to the participation and agree to assume full responsibility for any injuries or damages which may occur to the above-named children.
  2. You acknowledge receipt of, understand, and will comply with all terms and conditions of Little Hoku Montessori Academy Parent Handbook.
  3. You, for yourself, the above-named child/children, heirs, executors, administrators, assigns, and personal representatives, waive, release and forever discharge and hold harmless Little Hoku Montessori Academy, its officers, directors, employees, agents, successors and assigns, from any and all claims, actions, liability causes of action, demands, judgments, rights, costs, and expenses of whatever nature and kind you may have or may hereafter acquire, including but not limited to illness, personal injury, death, property damage or loss arising out of or in any way related to or connected with Little Hoku Montessori Academy or your participation in the school, except for those damages arising out of the intentional act of Little Hoku Montessori Academy.
  4. You will indemnify Little Hoku Montessori Academy, its officers, directors, employees, agents, successors and assigns from all claims, liability, causes of action, demands, judgments, rights, costs and expenses of whatever nature or kind brought against Little Hoku Montessori Academy arising out of or in any way related to or connected with your participation in the school except for those damages arising out of the intentional acts of Little Hoku Montessori Academy.

By signing this document, you acknowledge that you are waiving any rights that you or the above-named child/ children may have if the child/children – is/are injured during participation in the school. You have read and fully understand the foregoing terms and conditions and, by signing below, voluntarily agreed to be bound thereby.
